2010 Registration
Registration Form
Registrations will be held from 10-2 pm at The Elgin Family Moose Lodge, 925 S McLean Blvd. in Elgin on the following dates:

February 6 - Earlybird discount $250 (includes 3 Patriot Cards)
March 6 - $280 (includes 3 Patriot Cards)
April 3 - $280- (includes 3 Patriot Cards)
May 8 - $280- (includes 3 Patriot Cards)
June - TBD

Note: There is an additional fee of $25 for Pom Poms.

Important change for 2010 Season!
Registrations will be closed after the last registration date.

Payment plans will be available at registration. Must be paid in full prior to first game of the season. We accept Visa, MC and Discover.
 
Multiple child discounts available.

Registration Fee includes a non-refundable $30 dollars for three (3) fundraising Patriots discount cards. A minimum of $30 is due at the time of registration for Patriots discount cards.

Patriot Card—In order to defray some costs, we are having each player purchase 3 Patriot Cards at $10 a piece. The Patriot Card offers the bearer one calendar year’s worth of discounts at many local restaurants as well as other useful services. In addition, the bearer can present the Patriot Card when buying South Elgin Patriot merchandise for a $5 discount off a $50 purchase or $10 off a $100 purchase. The cards can be kept, or sold to neighbors or relatives for $10 per card.

Please make checks payable to: South Elgin Youth Football (SEYF).
A $25 fee will be charged for returned checks.

If a child withdraws for whatever reason, refunds will be based on the following:

• Refund request before May 1 – Full refund on registration fee (excluding Patriots discount card fee).
• Refund request between May 1 and May 31 – refund of $125.
• Refund request between June 1 and June 30 – refund of $75.
• Refund requests after June 30 – no refunds will be issued.
  

Players (football and cheer) must come to one of the registrations for sizing of equipment and uniforms. If a child can't be at the registration, please get them registered. They can stop by one of the following registrations or, we can make arrangements for sizing at another date. The sizing is very important. This is how team level committments are determined, and uniform sizes are ordered.
