South Elgin Youth Football Waiver 2023

I/We the parents or guardians of the child listed on the South Elgin Youth Football application herby consent to his or her participation in the South Elgin Youth Football/Cheer program. I/We do expressly waive for us, and for our child, any claim against South Elgin Youth Football, The Chicagoland Youth Football League, the Village of South Elgin, their officers, directors, and volunteers, and any claim or claims for injuries or illness which may result from your child’s participation in said program. I/We further agree to defend and indemnify South Elgin Youth Football (SEYF), The Chicagoland Youth Football League (TCYFL), the Village of South Elgin, their officers, directors, and volunteers of and from any and all claims, actions, and lawsuits of whatever kind or nature which may hereafter be brought by our child, or by anyone purporting to act on his/her behalf, whether said claim, action, or lawsuit alleges negligence or other wrongful conduct against said South Elgin Youth Football, The Chicagoland Youth Football League, the Village of South Elgin, their officers, directors, volunteers, or otherwise. I/We further understand by that our child is hereafter covered (with the payment of his/her fees*) by excess accidental medical insurance for injuries incurred by them during practice or game activities, subject to a deductible of $100.00 where there is no other collectible insurance. I/We recognize this document as a legal and binding contractual agreement, have read and understood same, and sign name as our own free act and deed. Our acceptance in the registration form affirms our consent to the waiver listed above and provides as our electronic signature.
Equipment/Uniform
I/We understand that the equipment/uniform used by our child, which will be issued to them pursuant to the payment of their payer’s fee* is only loaned to them by the South Elgin Youth Football program. I/We will return the equipment when it becomes due at the end of the playing season, and any lost or unreturned equipment is the responsibility of our family. I/We hereby acknowledge that football game pants and/or cheer uniforms are only to be worn for official events and are not to be worn to practice. I/We hereby accept financial responsibility to reimburse the program for said equipment in the event the equipment is lost or otherwise not returned at the appointed time. I/We further understand that failure to return tackle football equipment may impact our ability to participate in the organization in the future.
Volunteering
I/We understand that all families are required to volunteer for one concession stand spot during the season per participant. This is required of all families, even those that are coaches. Additional opportunities include: fan shop, league events, chain gang, 4th of July Parade.
Refunds
I/We understand that all refund requests must be in writing and sent to SEYF, P.O. BOX 599 South Elgin, IL 60177 or e-mailed to Jean Cheloni at southelginpatriots12@gmail.com and copied to . All requests must be postmarked or e-mailed by July 1st.
There will be NO REFUNDS after July 1st! There will be no refunds for discounts for referrals or sponsorships, only the actual amount paid will be refunded. Additionally, refunds for football registrations will be minus $50, for the cost of the jersey ordered. Patriots Raffle Tickets are non-refundable!

WARNING: Protective equipment cannot prevent all injuries. A player might get injured while participating in football/cheer.
*A $25.00 fee will be charged on all NSF checks
